### 1. System Overview

The [Secure Community Referral Portal](https://referraldev-anhyb3gmavaafqcc.eastus-01.azurewebsites.net/ecf) is a dedicated intake interface powered by FieldWorker. It is designed to capture, route, and manage incoming and outgoing community referrals for any agency. The portal enables users to seamlessly initiate referrals to external partner agencies or capture internal referrals using customized form templates, ensuring end-to-end visibility and structured data capture from intake through the referral lifecycle.

### 2. Step-by-Step Workflow and Interface Components

The portal features a streamlined, three-step workflow to guide users through selecting a destination, entering core client data, and launching the appropriate digital form.

#### Step 1: Select Recipient & Form Type

This section dictates the routing logic for the referral. Users must select one of three distinct pathways:

* Option A: **Search by Company Name**: A dynamic search field that allows users to query and select registered partner organizations by name (e.g., typing "an" filters the available directory). This is ideal for standard inter-agency transfers.
* Option B: **Enter Company GUID**: A precise text input field (`Enter GUID...`) designed for users to input a unique, system-generated identifier for a target organization. This ensures exact-match routing and is particularly useful for external organizations within the broader FieldWorker ecosystem.
* Option C: A generic or Custom Form: A prominently displayed action button (\[OPEN EMMANUEL CANCER FOUNDATION FORM]) that bypasses external routing. This option defaults to the Emmanuel Cancer Foundation's specific, customized intake template for internal processing or direct client intake.

#### Step 2: Complete Referral Information

Once a routing pathway is established, the user must provide essential identification details to associate the referral with the correct entities. All fields in this section are mandatory (indicated by an asterisk `*`).

* Referring Company / Organization: A text input field to capture the name of the agency, hospital, or community partner initiating the referral request.
* Client Name: A text input field dedicated to the specific client or patient the referral concerns.
* Client Address: An auto-completing location search field (`Start typing to search`). This ensures standardized geographic data entry, which is critical for matching clients to localized care services and coordinating logistics.

* Action Button: The \[OPEN REFERRAL FORM] button finalizes the initial routing and data capture steps. Clicking this button transitions the user into the comprehensive digital form corresponding to the choice made in Step 1.
* Submission Guidance: The interface provides explicit instructions stating that once the generated form is complete, it can be submitted securely through the digital system or printed for manual, offline submission.

### 3. Integration with FieldWorker Referral Management

The portal serves as the front-end intake mechanism for FieldWorker's broader referral tracking system, which provides several backend advantages:

* **Centralized Tracking**: Referrals initiated through this portal feed into a live, centralized dashboard, allowing organizations to track turnaround times, identify delays, and manage the referral lifecycle without relying on fragmented emails or manual follow-ups.
* **Ecosystem Connectivity**: By supporting both internal transfers (Option C) and external agency routing (Options A and B), the portal supports continuous care coordination across different support coordination teams and community resources.
* **Data Security and Compliance**: Information captured via the portal, including client demographics and specific needs, is processed over encrypted channels with secure access controls, ensuring that care transition data remains compliant and protected.
