Report Overview

The Active Customer Report, found under the Administrationarrow-up-right tab, provides a real-time, comprehensive census of every client currently receiving services from your agency. This report is essential for demographic analysis, state reporting, and maintaining an accurate active caseload. The Active Customer Reportarrow-up-right consolidates all primary data points from the Customer Profile into a single exportable grid. Unlike the "Status Report," which includes inactive files, this view is strictly filtered to show your current live census.

Column Definitions

This report contains the most extensive list of columns to ensure all required audit data is available at a glance:

Column Name

Description

Customer ID

The unique internal database identifier for the client.

Customer Name

The full legal name of the client.

Gender

The client's gender as recorded in their profile.

Customer Address

The primary residence on file.

Customer Phone

The primary contact number for the client.

Customer Email

The primary email address for the client.

Customer Acquired Date

The date the client originally started services with your agency.

Renewal Date

The date the next annual review or plan renewal is due.

Medicaid Eligible

A Yes/No indicator of current insurance status.

Medicaid ID

The client's unique 12-digit Medicaid identification number.

Waiver Program

The specific state program (e.g., Supports Program, CCP).

Primary Diagnostic Code

The primary ICD-10 or diagnostic code on file.

Medicaid End Date

The expiration date of the current Medicaid eligibility period.

Agency Assign Date

The date the client was officially assigned to your agency's care.

County

The New Jersey county of residence (used for regional rate adjustments).

Last Annual Visit

The date the most recent annual face-to-face assessment occurred.

DDDID

The Division of Developmental Disabilities identification number.

Sign Page Date

The date the most recent service agreement/plan was signed.

DDD Tier

The assigned budget tier (e.g., Tier A, Tier B) for the client.

DDD Plan ID

The specific identifier for the current ISP/Plan in the state system.

Grid Control Icons (Top Right)

Use these tools on the Administration Reportsarrow-up-right page to manage this data-heavy view:

  • Columns: Because this report has many fields, use this icon to hide non-essential columns (like Email or Gender) when performing a specific audit on Medicaid IDs.

  • Filters: Quickly isolate clients within a specific County or those assigned to a specific Waiver Program.

  • Density: Set to "Compact" to view as many clients as possible on a single screen.

  • Export: Use this to generate a full agency census in CSV format for state annual reporting or internal meetings.

  • Save View: If you frequently use this report for "Tier" analysis, save a view that places the DDD Tier and Medicaid ID columns at the front.

Administrative Best Practices

  • Clean Census: Regularly review the Medicaid End Date column to identify clients who may be at risk of losing eligibility.

  • Geographic Planning: Filter by County to help your scheduling team assign staff to clients who are geographically close to one another.

  • Audit Readiness: Keep a "Saved View" of this report that includes only the columns required for your specific state licensing body (e.g., Name, DDDID, and Last Annual Visit).
