Report Overview
This report consolidates all data related to the end of a service relationship. It is primarily used to ensure that terminations are documented with a valid reason, which is often a requirement for state audits (such as NJ DMAHS) and internal census management. The Client Termination Report, found within the Administration Reports tab, is a critical compliance and operational tool used to track and audit the discharge or transfer of customers from your agency's care.
Column Definitions
The report grid contains the following specific columns:
Column Name
Description
Customer ID
The unique internal database identifier for the client.
DDD ID
The Division of Developmental Disabilities identification number (specific to NJ Medicaid/DDD programs).
Customer Name
The full legal name of the client being terminated.
Customer Status
The final status of the client (e.g., Terminated, Discharged, or Inactive).
Transfer Out Date
The official last day of service or the date the file was closed.
Transfer Out Reason
The specific justification for ending services (e.g., Moved out of state, Switching providers, Deceased, or Goal Met).
Key Features & Actions
Audit Trail: This report serves as the primary evidence during an audit to show why and when a client stopped receiving billable services.
Filtering by Date: Use the Date Range Picker at the top of the Administration Reports page to view terminations for a specific month, quarter, or year.
Exporting Data: Like all administrative reports, you can use the Export icon to download this list as a CSV. This is helpful for sending "Discharge Summaries" to state coordinators or MCOs.
Empty State: If you see the message "No rows" (as seen in the current view), it indicates that no customers have been moved to a "Terminated" status within the selected date range.

The Administration Reports section includes a standardized set of interactive tools located in the top right corner of the data grid. These icons allow you to customize, filter, and export your report data without changing the underlying database.

Saves your current configuration (visible columns and applied filters) so the report looks exactly the same the next time you open it.
Actionable Tips for Administrators
Customizing for Audits: Use the Columns icon to hide internal IDs and show only the "Transfer Out Date" and "Reason" before hitting Export for a state auditor.
Rapid Search: Instead of scrolling, use the Filters icon to type a specific Customer Name or DDD ID to find a single record instantly.
Starring Reports: Notice the Star Icon next to the report title; clicking this will add the Client Termination Report to your Favorites sidebar for one-click access.
