Answer the questions to help us customize your account

• Do you have a primary person who looks after you?

• If yes, what is their email address? Check if this email is already in our system. If so, send a request that this patient wants to add them as a caregiver. If not, create an invite for this person to join the Ability Hub.

• A few basic questions to start building the patient profile.

  • What is your relationship to <name>?

  • What challenges do you face in your care?

  • Since when did you start needing care?

  • What is your living situation?

• Have a message that welcomes the patient and ask them to continue to build their own profile, and invite other people in their care circle.

Identify if you are a patient.

Do you have a primary caregiver?

Have the message that you now have an account on the Ability Hub app.
